Pilates Studio General Liability Waiver and Release of Claims

Assumption of Risk

 

I, the undersigned, hereby acknowledge that I have voluntarily chosen to participate in Pilates classes, training sessions, and/or activities conducted at Aria Pilates Studio , including but not limited to sessions involving Pilates reformer equipment, mat work, props, and other apparatus. I understand that Pilates, like all physical activities, carries inherent risks, including but not limited to:

● Muscular soreness, sprains, or strains

● Accidental falls, equipment malfunction, or misuse

● Aggravation of existing injuries or medical conditions

● In rare cases, serious injury or death

 

I voluntarily assume all risks associated with my participation, whether known or unknown, foreseeable or unforeseeable.

 

Medical Disclosure and Responsibility

 

I certify that I am physically fit, have no medical condition that would prevent my full participation in Pilates sessions, and have consulted with a physician as necessary.

I agree to disclose any relevant health conditions, injuries, surgeries, or limitations to Aria Pilates Studio and/or my instructor prior to participating. I understand that Aria Pilates Studio instructors are not medical professionals and cannot diagnose or treat medical conditions.

 

Release of Liability In consideration of being allowed to participate in classes and use the facilities, I hereby release, waive, discharge, and covenant not to sue Aria Pilates Studio , its owners, instructors, employees, agents, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury (including death), or cost that I may sustain, whether caused by negligence or otherwise, while participating in any activity or while on the premises. This release applies to myself, my heirs, assigns, personal representatives, and next of kin.

 

Use of Equipment

 

I agree to use the Pilates reformers and all studio equipment only as instructed. I will follow all safety guidelines provided by Aria Pilates Studio and understand that failure to do so may result in injury for which I accept full responsibility. I acknowledge that improper use of equipment can increase risk of injury.

 

Photography and Media Release

 

I grant Aria Pilates Studio permission to use photographs, video recordings, or other media taken during classes for promotional, educational, or commercial purposes unless I opt out in writing.

 

Acknowledgment and Understanding

 

I have read this waiver in its entirety and fully understand its terms. I understand that by signing this document, I am giving up substantial legal rights, including the right to sue. I acknowledge that I am signing this waiver freely and voluntarily.
